Once it's on yes it will keep the water inside. Noise won't be an issue. The only pumping done is when you put it on. No more is needed. You "set it and forget it". Seeing as how there are a low number of men reporting issues with the base rings coming off or having other issues (''disintegrating''), I have to assume that just like with any other manufactured device where there are humans involved, some occasionally slip by quality control. That is normal and to be expected. Most men of average IQ though should be able to fix this base ring issue on their own without having to pay for return shipping and then wait for a replacement. That's what I would do anyway. I've even thought of how I could replace the comfort ring if need be by using a stenciled and cut out piece of a neoprene mouse pad and some water proof rubber cement. LOL. Doesn't hurt to plan ahead.
