If you can only get into the large bell I don't think you can get this combo to work. I ripped about 3 sleeves and 2 condoms trying to get it work with the large bell. With the medium I have no problems, perfect. My glans are about 5.25 so most should be able to fit into a medium. It takes some time for me to stuff it in there but I can make it happen
