It works on it's own. It would be something you use in place of the phallosan in case you were not able to use it for whatever reasons.

Say you do stretches, then oh shit I forgot my phallosan at work. This is when the UJW would come into play or using @ nighttime in case you don't want to use the phallosan for night time extending.

Only use a UJW or sleeve/ETC when you're having issues w/ the phallosan. Hopefully that is never. You could get glans problems from the phallosan if you fuck up somewhere along the line.

Not nearly as intense, more of a method to heal in an elongated position to help cement length gains.
