If you work yourself up to 5D-BTC-BS you don't need to change anything.

That exercise is valid for at least 5 Inch increase in BPEL. I believe it's valid for even more. You have so much force, intensity and leverage in that position. Especially if you are performing them with LengthMaster 3, and are sitting on your knees to perform them.
You are stretching every angel behind you. Your shaft, ligaments and inner penis can be applied with maximum stretching force in this position.
