I have heard people refer to it as a comfort insert. The cushion ring does not attach directly to the pump. You place it around the base of your shaft and center the pump on top of it. It really is quite comfortable and pretty much eliminates any chance of 'ball-suckage'.

I'm temporarily in an inconvenient life situation right now, since I recently lost nearly everything in a flood and am staying with my mother until I get sorted out, so my PE is restricted to just manual stretching right now. It's the easiest thing to do with a small time commitment and no equipment.
