I used the medium duty off and on these days but when I first got the package I used it quite a bit everyday and I'm not sure about EL gains but it sure did WONDERS for my flaccid size. I've always got a bulge anymore and I love it. I'm impressed with how it has stayed that way and I haven't worn the medium for almost a year.
It does take some mastering to put on but if you relax and are patient you'll get the hang of it and then you'll wonder why it was so much trouble at first. One thing I learned the hard way was to not put on too much tension like being over anxious. This only gave me skin fatigue, so mild tension is the key IMHO.
I don't think you can say that erect gains automatically create flaccid gains. I think most people here would agree that's not the case. Flaccid is different. I do think the medium could create erect gains, I just am not in a position to log as many hours as I think it would require. I use the Bib because I like the feel of hanging and because vacuums are trouble for me. Although the Bib can wear my skin out too. That's why I continue "hanging" with the heavy duty. So in a nutshell, I hang with the Bib until I feel worn out from it, then I switch to the heavy duty and go (usually) a couple more hours non-stop. The combo gives great fatigue. I gained 1/2" within the last 90 days and also took a two week vacation during that time.
I also like the heavy for travel. Someone else suggested it and I tried it. Much more compact and easier to bring than the bib + weights . And it doesn't trigger the metal detector because it's all aluminum and plastic.

I can say that since I added the heavy duty to my post Bib sessions I do not feel I can or want to go up in weight as fast as without it. I consider this positive since I think more weight is just more trouble and possible injury. Therefore the lower I can keep the weight and continue to gain the better. I think since the heavy duty is a mild to medium force hanger (in effect) I am sustaining fatigue from my Bib sessions without having to use the Bib. Partly this is due to the multi-hour sessions with the heavy duty and the fact it can pull up to 11 pounds.
Also, the 90 day gain has taken me a year to dial-in. I hope that I've now finally found EXACTLY what it takes to make me gain. We'll see what the next 90 days brings.

The last three months have seen over a half inch and possibly 3/4 inch. But at the moment that last 1/4 sort of comes and goes. Measured it again today... so at least it's trying!!
