I'd imagine new veins would have to grow. Your penis would have to build whatever it requires to retain all that blood that you are constantly engourging it with. I'd imagine that thing would be veins primarily. I cite myself, Shane Diesel, and everyone else that seems to have gained girth, because you really start to develop some new veins when you start gaining girth.
In my case, reduced jelqing has resulted in loss of girth. I might have gained a half inch from the months and months of stretching exercises. But the girth seems to have suffered.
I'm going back to my old standby--jelqing with a downward stretch at the end of each stroke.

Women must be able to reach the inner most rim (back side from the entrance) of her cervix in order to install a diaphram. This puts that most inner portion of the cervix less than 4" (and more likely 3" and a bit) from the entrance to the vagina. Therefore many penises over 6" are likely to be passing by the cervix.
