Ideally you should've stayed with 5 lbs for at least a week or two to accomplish the above and then gone up a pound or two per week. Any problems such as faulty wrapping, not tightening enough, not having the Hanger seeing perfect for you, etc at lighter weights will be compounded at heavier weights.
You seemed to jump up there way to fast. I haven't read all through this thread but I'm sure you ran into problems perhaps even numbing or injuries. Maybe your reaching a point where your tissues are more adjusted and now you're hitting fatigue. Are you hanging 40 min sets or 40 min total?
