save youself some $'s and get the regular X40 and add the VacuVin mod...from what I gather, the mod creates much more pressure than the X40X. I swear by the X40+VV

Isn't the comfort sleeve a lot better in the Xtreme and you're getting a lot of extras, right?

The comfort sleeve that came w/my X40 is very comfortable against my pelvis area and the sleeve portion extends inside the BM and beyond the gaiter so I don't get that awful bite. I can't comment on whatever else is included in the extreme, (maybe the hanger and case?) but even if I got the extreme, I'd mod it w/the VV

I couldn't stand the comfort pad but I only tried using it once. Got more uses out of my x30 pad but now I cant even use my ex thirty because it crushes my vas

I don't think you can mod the Xtreme.
A Hydromax Xtreme includes (c&p from the official page):
