Do you clamp at all? Where do you hold your penis when you are clamping? Maybe you just need to find an exercise in Mike's huge arsenal of dick exercises that focuses on base girth expansion. It has to be in some thread--has to be. For most guys, it's the opposite of your situation.
After you get a decent erection, I would just keep forcing the blood back down to the base, and then clamp it as far down to the very root as you can get it, right into your ball sack, and then hold it there with one hand. Clamp it real hard.
Take one hand and do that. Then take the other hand and start forcing the rest of the blood in the upper shaft down, down, down. Over and over. Keep your mind focused on expanding your lower dick. Then clamp it, let's say, at mid-shaft--maybe lower.
Picture your dick, thick, thick, thick from top to bottom. Visualization is extremely important.
Do you ever masturbate by pulling your hand down your penis shaft? Duhhhhh? Just kiddin, pal!! But grease it up good so you don't irritate it, and try this.
It's all about trial and error--trial and error. Keep trying different approaches, and you'll find the one that works and before you know it, your meatus will respond in kind.
