Kinda late to this post but have thought about it many times. I think alot of women or maybe most women gauge their idea of erect size by looking at the whole package, i.e from the bottom of the scrotum sack to the top of the glans. They're not just measuring the pecker in their minds but the entire package when erect starting at the nuts. That's my take on it.
 
I think woman see size in a similar way to men. I think there is always a bit of anxiety when we see someone naked. I also think that the world is programmed to see a penis as small, normal or huge and the measurements that go with these name usually fall into pre-determined numbers. For instance a woman sleeps with a man, in her opinion the guy was huge, she never sleeps with the guy again. As time progresses I think the title huge eventually gets slapped with a number; she may say "He was like 14 inches" even though the guy may have been 7".

I'm 6-3, 230, not as big as you are, but I definitely feel you on the body size point. It's all relative. If you have a larger frame, your piece tends to look smaller, all things equal. Because women don't live with dicks between their legs everyday like we do, they don't obsess over how many hand widths long a dick is, how far it stretches to someone's navel, etc., etc., etc. So they are just not very good at estimating length. In my experience, they are even less good at estimating thickness in terms of numbers.
