I assume you mean flaccid stretched length? If so, in a completely flaccid state, you grab the glans at the coronal ridge, and stretch out your penis perpendicular to your body with one hand, while you gently press the ruler against your pubic bone at shaft-level with the other hand. You measure to the very tip. The amount of outward force should be around 2-3 pounds.

To be more specific, the ruler should be placed in the center-top of your shaft, gently pressed against your pubic bone. Do it this way everytime, for any bone-pressed measurement.
The only measurements I care about are BPFSL, and BPEL.
If BPFSL is increasing, then I know BPEL gains are coming. If BPFSL is not budging, then I would have to refine my routine, either by adding more intensity, or time, or both. Therefore, I check BPFSL once a month, with a set goal of 1/16" gain.
I personally don't care about NBP measurements of any kind, because as long as BPEL is increasing, the rest will follow.
The fat pad will shrink with a good diet and exercise, and has nothing to do with PE itself.
Some guys like to keep track of flaccid length. I don't see the point. After a session, it looks a little longer, which is great. And then I go out in the freezing cold to buy some groceries, and it is not the same anymore. It constantly changes depending on temperature, stress levels, etc. So why even bother keeping track of it? BPEL is what counts at the end of the day. I really don't care about the rest.

@Grandmaster - This is the reason I'm looking into to creating a heated wrap, but it may or may not be necessary to make gains.

A heated wrap can help relax the tissues, allowing you to stretch them further. Doing this can also help prevent injuries. So it is useful while you are stretching to apply heat. Many like doing it.
Personally, I never did more than a 5-minute warm up, and have gained just under 2 inches in BPEL. These days, I don't even bother warming up. With an extender, because the tension is not that high compared to other methods such as manuals, the penis gradually adapts over time, so the risk for injury is actually very low. You can argue that using a heating pad is always better, and theoretically you would be right. But I just do not feel it is necessary in my case.
If you are not under traction, or about to be under traction, then using a heating pad would be completely useless. Your tissues need to be stretched passed the marginal stretch, and in fatigue for a certain period of time, on a daily basis, in order to make gains.
If you are not stretching, wearing a heating pad would be the culinary equivalent of trying to cook without a fire. Nothing will happen.

Yeah! It's working for sure. Doing really intense bundled stretches with my broomstick. My intensity really kicked in also. Interesting how much length this will bring me in 6 months.

I've never warmed up for PE before. But my country is a hot place to live in so I understand the need for the heat pad for others.

Yeah, I see what you mean.
I could not live in a country with a very hot climate. Where I live, it is cold most of the year, so that is what I am used to. In summer, if I am not at work, I spend most of my time near my air conditioner, and only go out at night. I hate the heat. I hate being sweaty, hot, and breathing in hot air. I do not feel good under those conditions.
