So near the bottom of the head just under the head there are little tiny white bumps here and there, I don't think it is PPP though. Of all started happening after the extender use. Also, the bottom of my head hurts after using the extender for just 3 hours a day. What could fix these problems? Possibly hydrocortisone cream?
 
It could be PPP. I´ve had it since I was a teenager and I think it gets worse when its either very dry or when its moist like after a blowjob when I dont dry it afterwards.

However I dont think its common for adults to suddenly develop PPP. Could be a fungal infection from a unclean device. If it starts to itch then thats definitly it. Have you been using any lube with your other exercises where mold might have developed in the lube?

It's not really itchy at all. I believe that I have a couple PPP spots on the underside of my glans. I'm only 18 lol, so I'm still not really an adult yet so it's more likely that I could acquire them. but ya I'm pretty sure the dryness and pressure from the comfort pad is what might be causing it.

If it is PPP then dont worry about it. I´ve never had any girls comment on it or react to it.
 
PPP is not something that happens due to something you did, they are usually something men have their whole life. I do not think you have PPP but a picture would allow me to really diagnose this better. It could be minor blistering but without seeing it is hard to say for sure. If they are somehow PPP there is a cure for it so no worries there.

What do you suggest that I do about the soreness while in the extender DLD?

I'm using the noose method with the comfort tube wrapped around it. Sadly that's the only way of strapping myself in cause I lost everything else (only got the noose and the strap and I lost the strap) I strap myself in right under the head, fairly tightly.

Ok, go to the shops and buy yourself two things: 5cm (roughly 2 inch) wide cotton crepe bandage and some velcro cable ties (or roll of velcro).

Wrap the crepe bandage around your shaft, under the glans and use velcro to stop it from unraveling then apply your extender. Also, look into using a velcro strap to secure your extender. It's kind of hard to explain it but there are threads on this if you do a search (or if someone is nice enough to link to a thread about it)...

Your dick will still alter its appearance after a while and you'll still get odd bumps sometimes and maybe a thicker glans ridge, it all depends how long you wear it, how often etc. It's something you'll need to work out with your dick as everyone's different when it comes to pressure tolerance, recovery etc...

Have you tried the Velcro Tape modification? This is pretty cheap and much more comfortable than the noose. The VLC would be best but I think the Velcro will also be good.

Soreness could be due to your dick not properly warmed up or getting cold during your workout. Warm up your dick constantly.
