Is the reason you measure to nearest quarter inch because zero starts a quarter inch after the ruler starts?
 
I measure to the 16th of an inch. I also measure very often (sometimes daily) and keep a sort of running average in my head, to try to account for variance in erection quality.

Another thing I do is to measure every possible way. BPEL (sitting), BPEL (standing), NBPEL (sitting and standing), flaccid, flaccid stretched, erect mid-shaft girth, base birth, post-workout size (for different kinds of workouts), and I'm sure there are some I've missed.

The reason is because there is not a perfect correlation between those measurements. For example, my "standing" and "sitting" length do not change at exactly the same rate at exactly the same time. The biggest factor is probably a lower exit point as my ligs get stretched. But I've seen one measurement get bigger and the other remain completely unchanged.

This may not work for everyone, but it works great for me. Just measure consistently and you're all good.

Edit: I forgot to add that I also cut the end of my ruler so it starts at 0.

That is the case with many rulers and for a long time I was measuring .25" less than I was. I shaved that extra quarter inch off with sand paper.

Ruler, for sure. I used a metal tape measure for a while, and the risk is that it will snap when you press too hard. I always felt like I was on the verge of cutting my dick in half with the metal.

And there's not a universal doctor measuring device. I don't think we should be paying too much attention to the way they do it either. I can guarantee you that we (brotherhood, forum members, PE community, etc.) know a hell of a lot more about measuring penises than doctors do. There was no cock measuring lesson in medical school.
 
Ruler for measuring length and a tape measure to measure girth. Very simple.
 
You measure from as deep in the fatpad as possible
