I believe he has had ALLODERM or Fat grafts to add thickness.If you look at the photo top right you can see the base of the shaft tucks in (just above the balls). Also the lack of vascularity because the Alloderm/fat grafts are placed between the the skin and the actual shaft.
 
Look at the guys head. He's probably around 6 to 6.5" normally.

Then notice the shaft doesn't have hardly any veins. Defintely pumped for quite a while and probably took viagra (or some injectible) during the pumping session so he could be ready as soon as he took the pump off.
