Yes, it looks like you have to add another 1 inch bar or half a inch bar. Once the screws/inside bars are to the top are showing, its time to add more bars.

I thought the comforts strap was supposed to sit behind the head. In your pictures, it appears to be half way. And the countered side of the strap faces away from you, so your head can rest nicely once strapped in.
 
A statement I usually rift on, is as long as you are getting a good stretch, you should be fine.

I have read from different sources that you generally want to stretch your penis so that you are 1 inch longer than your optimal erection size. for me its not even close, my erect length is 5 inches, but stretched with the [words=https://shop.mattersofsize.com/products/sizegenetics-penis-extender]SG[/words] on im still under 5 inches, and it feels like any more stretching and i'd rip

Same here, I'm not that much more stretched if any past EL...
 
Also, the type of attachment will give a different stretched length. For instance, the [words=http://TLCTugger.com/MOS]VLC[/words] will not stretch to the same length as the velcro strip, but both are productive stretching point. As stated above, as long as you are getting a good stretch and making gains you are good. With that said, it is important to be sure you are "getting a good stretch" and what to look for in this is fatigue at the base, increase in flaccid stretched bone-pressed length, need for new bars or adjustments on the [words=https://shop.mattersofsize.com/products/sizegenetics-penis-extender]Extender[/words], etc.
 
Shucks I hate hi-jacking threads, but with what you said DLD, I don't think I've ever felt fatigue at the base in my [words=https://shop.mattersofsize.com/products/sizegenetics-penis-extender]extender[/words]. With the [words=https://officialhydromaxpump.com/?uid=6&oid=2&affid=98 ]X40[/words] very much so, but with the [words=https://shop.mattersofsize.com/products/sizegenetics-penis-extender]extender[/words] I usually feel the burn just under or just below the comfort strap. Am I doing it all wrong? I mean I am adjusting through the day as well, even close to adding 1/2" bars because of thread showing.

I would not say you are doing anything wrong, you are simply mastering the process. It takes practice but eventually your mastery will resolve these issues.
