(see what I did there? lol!) its URL brings up a surgery site. Plugging the picture into TinEye pulls up a Wiki page on it.
That's like 4,300 years old for the original version of that picture.
As for different types of Penis Enlargement technology, the concept of a persistent
traction device - like with an
extender (being self-contained from the body) - might not go back that far, but manual stretching and jelqing probably do.
There are several ancient references to 'preparation of the member by placing a heavy burden' and by way of a 'heated massage' etc, but I've never seen any dates given for them to see just how far back it could be reliably traced. There's a Biblical reference in Ezekiel_23 of multiple people from Egypt and Chaldea (Babylon/"Iraq") "whose flesh is as the flesh of asses, and whose issue is like the issue of horses."
Any sufficiently broad group of people is going to have examples at the ends of the curves of a few small and a few large penises (with most trending towards the middle as Average). Horse-hung dudes being in a ready supply from any population would be outside of the averages and indicates knowledge of some form of Penis Enlargement technology. Chaldea for that example is like 600BC, but it's probably even older than that.
