I have finally developed a method for wearing the extender and a wrap (a variation of velcro mod) that allows me to put in a lot of time without discomfort. I finally have a method that eliminates irritation/skin inflammation due to friction at the base, along the shaft, and at the wrap. After years of messing around with the extender and dealing with constant discomfort, I now can easily wear the extender for 2+ hours, and have even done 3 hours before rewrapping. Even then, I just rewrap to encourage good blood flow. Discomfort is not an issue. I now can achieve 5+ total hours a day without discomfort.
So with my new method, all I really feel is some dull aching after about 2 hours. There is no real pain, and no discomfort. I use a tension that coincides with the 2nd to last marking on the rods which house the springs.
My question is to those that have achieved results with extenders: Is pain and discomfort necessary to make gains? If so, how much?
