I have got a jes-extender or vimax extender, but i had some comfort issues when wearing it, so I have modified it, allowing me to wear it for 12+ hours a day at 1200g tesnsion as the manual recomends for best results.
First off, I have covered the part of the base ring that goes agains the underside of your base with Dr. Scholls moleskin, which is very soft with adhesive on one side, so iritation there is gone.
Next, I replaced the silicon noose with a nylon cord noose, using two cordlocks to adjust it. I got these supplies from a local camping store.
Third, and most important, is I use Curad scar therapy pads to cover the area of the glans that the noose holds onto, because that is the area most prone to irritation. The pads are clear polyurethane with an adhesive side. I just place then on there, then apply the extender like the manual says. Here are the pics.
I have only been rearing it for a week or so, but my ligs are looser already, and my erections have improved.
