Hey man. The first part of getting great customer service is being a "great customer".
I'd write them an email, and PM Dave here on the [words=http://www.mattersofsize.com/join-now.html]Mos[/words] forums too. Let them know,??? they missed some items in your package. Most of the instructions can be gleaned from viewing the instructional video on the [words=http://www.mattersofsize.com/mosdvd.htm]DVD[/words]... that's the instructions...
Also understand, this product is SO good they are having insane success as a fairly new company and are growing at a crazy pace. Some low % of errors happen to the very best companies in every industry.
You I'm sure have an error or failure rate doing your job too. You fix them and move on, right? Give them a chance to make it right. This is called " do unto others" principle. Would you like your boss to just fire you the first time you fuck up?
So maybe cut them a bit of slack, ask for service by email, and Then report the results here to us all.
I'm sure they will want to send what they can to correct their mistakes and keep your business.

The sales have been extreme and they're having a hard time ramping up. Not surprised if someone at the fulfillment center missed tossing something into your box. Random % error rate in every company, but to be more expected when they get hit with extreme sales spikes.
