OK - have started using it today and am keen to see what difference it can make. I have taken my measurements and will try and resist measuring again for a month!

One quick question - am I supposed to stretch past my erect length or past the flaccid length? I tried going past the erect length but the head keeps slipping through the noose (when it is flaccid).

As far past your erect size as possible. Some guys will only get slightly past and that is fine as long as the stretch is always pushing the bar.
