I don't know if this comment applies to all women. I've probably experiemented on my wife the most over the last 15 years, but what I can tell it doesn't matter what the size (small or large), she could not achieve a vaginal orgasm (I've tried all different dildo sizes). What does happen is that when she plays with her clit and we have sex she has a clitoral orgasm. Its much stronger when we use a large extension. Other women I've been with also only have had a clitoral orgasm.
