Well- it really depends (was this inspired by the "Gangsta Urologist" thread?). I have no experience with fast jelqing, but what I believe is the plus of slow jelqing is the fact that you're letting your penis stay more in the expanded state. I would love to see other people's answers, though. :)

Slow will give you better expansion and is more likely to give you faster results, but do keep in mind that, because of that, you shouldn't do as many repetitions as you would with fast jelqing. When you jelq slowly, you're putting more pressure on your penile tissue, so you mustn't overdo it.
 
Yes, I believe you could use time as a guideline. You should notice that, with slow jelqing, you'll be doing about half or 2/3 of the repetitions you'ld be able to do if jelqing fast in the same amount of time (I hope this sentence made sense).
However, depending on the intensity of your jelqing motions (grip strength, erection level, etc.), you should look at how your penis reacts and look for signs that you should stop for that session. For instance, if I notice that, during the session, the dreaded red spots are starting to appear on my penis' skin, I stop and that's the end of my session for that day. It's one way my body has to tell me "enough already, you're pushing too hard".
 
Yeah, definitely watch to make sure you don't over-do it. I would do fast jelqs one day, measure temporary expansion, and then the next day do slow jelqs for the same amount of time, and measure temporary expansion again. Whichever gives you more expansion should be more effective, as it's a good indicator of how effective your girth work is.

Note that you should use the same grip strength and if your eq is lacking one day that may throw off your results.
 
A slow jelq is a jelq but a quick jelq might end up being a jerk and then you got a mess:)

Best way to jelq is in about a 1-2 second strokes, never missing time in-between, being sure that most of the blood remains in the penis for the next stroke. Always kegel during jelqing to increase blood in the shaft and to practice muscular strength in the pelvic muscles.
