I work with photoshop on a day to day basis and I can say it's definitely fake. No I'm not going to explain why I say that, it's just I have an eye for spotting fakes. Really though enlargement and shrinking of body parts (not just genitals) is all the rage right now, and there are many easy-to-follow tutorials out there for photoshop that anyone that owns it can easily, very easily make fakes like this.

ok as example here is a fake I just made, took me 5 minutes in photoshop, if I were to sit down for an hour or more, covering every little detail, and if I had a higher rez starting photo, I could probably produce more realistic results while at the same time making the size even bigger than what I was able to do here..

left is before, right is after.

Not only can you make the dick in the photo bigger, but you can make the guy smaller/thinner (to give the illusion of a bigger dick), as you have probably noticed that is what I have done on top of stretching the dick out.
