Yes, it is about the bell being too small. If the penis is bigger than the bell it will look for space where it can. So it expands at the flange of the bell, exactly where the condom is wrapped around the chamfer. A proper sized bell (dashed lines is the cock) would emerge from the bell without expansion. So in the bell that is too small, the penis is overexpanding which then causes problems (same as for exhaust gases coming from a nozzle).
The only way I found to cope with this problem is to use a normal condom as protector
cap or sports tape. Since protector caps never worked for me I am pulling the foreskin to the front so that it covers the glans, then I put one sports tape to hold the foreskin together. Then another tape lateral to the first one. If this tape is long enough it will cover the section where the bell ends and then the skin doesn't get squeezed. However it is painful to remove the tape so it is not a good option.
Actually it gets squeezed when rolling down the condom. If you get it right at the beginning it is less likely to happen later. If you roll the condom fast, it will strikes the skin and the skin will move out of the way, mostly just in between bell and condom. Therefore it is required to pull the shaft skin after rolling down the condom. But then again some users will pull the glans out of the bell and things like that. It is a bit tricky. Hanger condoms being nontransparent it is not easy to spot squeezed skin. However after a while it works without visible inspection.
