1- I dont think it should be a problem- it is just ink beneath your top skin layer. I see no reason for it to be a problem. I do not have any tattos and therefore can't tell you for sure. For this- I think someone with a tattoo down there needs to tell you...

2- It is either physical or mental.

Physical- you like the feeling of your grip better. You grip tighter and it stimulates you to a level where you consider it better than sex.
Mental- there can be MANY reasons here- you're not really into the chick, there might be a mental barrier of built-up stress that gets in your way to cum, you are not aroused enough etc.... I have read of people who have abused adult entertainment and have desensitized their penises to normal sexual intercourse.

I can't tell you whether it is physical or mental, not what it actually is- but following the logic you should be able to analyze and find what the problem is yourself. Just ask yourself- "Am I properly aroused when I have sexual intercourse" , "Do I have any problems in my life that are putting too much stress on me and therefore lead to my inability to cum" , "Am I making any associations with anything bad I have experienced during sex" , "Do I like it more when its tight or when its a bit more wide" , "Is my hand tighter". I think that would help you find the main reason.
