In my experience, it is a number of factors. But if you examine all these factors, the only one that many guys are likely to miss/forget/be ignorant of is: Style or Strategy in sex. For example, someone might say "Not enough foreplay"... and while a definite factor, it is unlikely that none of these men failed to warm her up and get her hot before beginning intercourse.
