The problem with Sickle Cell (largely being a black disorder) is, yes the penis can become quite large but in most cases impotence makes impossible to use. There are rare cases (one that was just posted last week) where the man did not follow medical protocol (carbon-dioxide injections) to relieve the erection. He grew to a large size of 8 x 6 " and is still fully functional. This is usually not the case, in most cases if therapy is not taken the patient will become impotent.

I agree with DLD. For the most part, it lies in genetics. Warmer climates fall towards the theory of heat dissipation through larger surface area.
