There's only one way to go-

Pull the fuck out of it at every opportunity. You'll get some pain BUT you'll gain fast (I got 1/2" in less than four months of less than full commitment!)

Depends on whats comfortable for you, I wear mine both ways.
But I do find it more beneficial from the way it is pictured.
