Is this a fracture? Please help!

I tried this jelqing thing at home when I was a kid in high school and I had developed OCD by being put on ADHD meds in elementary school when I don't have ADHD...On top of that my parents were screaming at me every day, making my life a living hell, contributing to mental health issues...so anyways at some point this became a semi-intentional self harm thing and it went on for years off and on.

I can still get hard, but due to the pain fo activity, I can't have sex. It feels like an open wound being messed with if I try to masturbate unless it's with special lotion. I can't even masturbate more than like 5 minutes at most if I'm lucky and take extra care.

I'm finally off the ADHD medication (Concerta) and it's been 6 months not taking it, and my penis is looking healthier but still the pain is horrible when I even slightly have to urinate. I'm scheduled to see a primare care doctor in about 2 months, and expecting to be sent to a urologist, but I'm looking for a second opinion in the meantime.

I'm 23 and decided to seek help because A) i can't have a sex life and equally B) i can barely perform at work and due to the private nature of my issue I can't explain it, and I am constantly anxious to the point of shaking (thanks to the caffeine which I'm still trying to reduce). Any input would be appreciated

It looks more like possible abrasive damage than a fracture to me. If you fractured it you'd probably know, ie not able to think about anything else level of pain along with severe discoloration and swelling. If you have to ask whether it's fractured: it most likely isn't. Seeing the Dr is def your best bet, people here have a lot of experience but they can't diagnose or examine you personally. Try to relax and use some otc meds/numbing in the meantime, I know it probably feels like the end of the world (been there) but it definitely is not.

Feel for you man! Did you get that from jelqing? So it doesn't want to heal?

There is an ointment called: Locobase ® REPAIR and I use it between stretching, jelqing and Bathmate sessions, it works by protects and restoring the skin barrier. It's made a big difference for me to keep up the PE.
Maybe this will help you.

Also if you want to jelq in the future, you can use baby oil and/or Vaseline.

Related to your OCD issues.
I recommend you to demand cognitive behavioral therapy from your healthcare provider.

Then you get professional help to change your behaviors (so you feel better), as indirectly help you with your feelings about yourself, and the relation to other people. You start to see that nobody is perfect, we are just different from each others.
