In my experience, it is a number of factors. But if you examine all these factors, the only one that many guys are likely to miss/forget/be ignorant of is: Style or Strategy in sex. For example, someone might say "Not enough foreplay"... and while a definite factor, it is unlikely that none of these men failed to warm her up and get her hot before beginning intercourse. Pussies are much more complex than penises in my opinion. They differ drastically. Trying for various techniques and cocks is not the right way to go about it in her case. Making her comfortable with you, gaining her trust, is the ticket to having her let down her guard and let herself be carried away into an orgasm. Even then there arent any guarantees.
