The rule of thumb would be when you need it. I would recommend keeping a light tension (2 sections showing). Maintain that tension as the penis stretches. Eventually days - weeks, as you increase the tension you will start seeing threads. If you start seeing threads, it is time to add a bar. Now keep in mind, use the bars you have to do the slightest increase. For instance, when I add a bar, I only increase by 1/8 - 1/4 inch max. 1/4 is pushing it. This way you are still only placing light demand on the penis and nothing hard core. Light demand = longer wear time and faster gains.
