At the foundation was attack the tissues very aggressively if needed, back off and let rest and repair work. I'm currently experimenting with a modified, in light of new info, and toned down version. So far around 0.5 inch gain in BPEL in one month that as of 3 week's into decon has not lost even a 0.1 cm. Though around 1/4 inch was reclamation of lost gains the 20 months before that.
I hope you get the gains you are seeking. I'd recommend heat the first half of your sets and if you plateau rest. DLD hung with very heavy weights for a time too.
