As brother 9.5inchpro said, it's the same. The fundamental difference is the traction slip over time when using the extender.
When you're hanging, that static weight will remain at 3lbs no matter what if you're hanging it straight down, straight out/up/sideways using a pulley system.
But for the extender, the extender's springs relax as your penis lengthens, reducing the static traction load down to 1lbs or so. If you're using longer extension rods, the rods play a role in pushing the tissues apart at the mounting points (base of the penis and the holding point, like the vacuum cup or the straps) and keep the traction load up. But the reduction of traction load still occurs over time. Rather than at 3lbs, it slowly reduces down to 2lbs, then 1lb.
