The testicles are located in the scrotal pouch to be 1-2°C cooler than the rest of the body. This makes them more exposed to trauma than the ovaries in women, which are protected inside the abdomen. Testicular injuries are relatively frequent as a result of contact sports such as football, aggressions, or traffic accidents, especially motorbike accidents (due to impact on the tank).
The scrotum is a particularly painful area. Anyone who has suffered a kick knows what I mean... Why so much sensitivity? During embryonic development, the testicles descend from the abdomen into the scrotum, and during this descent they carry many nerve endings connected to the so-called vagus nerve that innervates the intestine. The sensation of testicular pain is perceived in the brain as a stomach ache, and the defence mechanism is the feeling of nausea, dizziness or sweating.

But in addition to being very painful, a kick to the testicles is very dangerous, as it can lead to testicular rupture. When a testicle breaks, a bruise forms around it that manifests itself as a significant increase in scrotal size associated with a lot of pain.

When faced with testicular trauma, it is necessary to go to the emergency room to be explored by a urologist and have an ultrasound scan performed. If the ultrasound shows a ruptured testicle or there are doubts, then it is an urgent case of surgery. The scrotum must be opened, the hematoma removed and the testicle evaluated. If a repair can be made, the testicle is sutured and preserved. But if it cannot be done because there are multiple rupture lines (testicular burst), then the entire testicle is removed.

A ruptured and repaired testicle can see its reproductive function altered for different reasons: a decrease in the production of sperm, obstruction of the ducts which carry the sperm, or the formation of antisperm antibodies which later make its fertilising capacity difficult. The testosterone production function is more resistant, and as long as there is viable testicular pulp it can still be maintained.

Just yesterday I operated on a broken testicle in a patient who had been kicked around by a discotheque keeper. In this case we were lucky and the testicle could be saved, but this is not always the case. If you practice a risky sport or profession, it is advisable to wear a testicle protector just in case.
In the event of any testicular trauma that produces swelling, always go to the emergency room!
