For TLC / VLC gear we use flaccid and erect glans width measured at the widest point, as shown here: Device Sizing | TLC Tugger Foreskin Restoration
We strongly recommend you make pics like the ones shown whether you study them yourself or send them with your order.

You would be better off with the VLC Hanger (formerly called VLC-X), which has a solid rod and collar with set screws, instead of the chrome tugging handle.
It's not a quick question. The VLC Tugger has a tugging handle which is optimized for straps, traction rigs (SG), and weights, in that order. The VLC-X has an aluminum rod which is optimized for weights, traction rigs (SG), and straps, in that order.
It's not quick because we have a...
Peyronies therapy should always include a doctor's care, but IMO using a tugger can do a better job for anti-curvature traction, because it pulls your WHOLE shaft from the end of the glans instead of grabbing your shaft below the glans.

You can stuff something maybe 1/2" long (like a chopped section of a ball-point pen barrel) into the holes in the extender's yoke. Although the result will be less stable, it will buy a little length.
A reminder, if you bought a VLC from August through October you can get an SG clip for...
You can PayPal directly to TLCTuggerService@GMail.com. The MOS offer is $55 plus $7 ship/fit charge. Also send your glans width (flaccid and erect http://TLCTugger.com/YSC-size.htm) to that address.

Any TLC Device can work with the SG. You may need to extend the reach of the exender with additional segments, but interface them with the SG we have SG Clip for Tuggers that have loop handles and the SGX Clip for Tuggers with a Rod or Tube instead of a handle.
As some guys have asked, YES I can make an SG Clip for the VLC-X version. I'll call it the SGX and I have it listed at the MOS Offer page: http://TLCTugger.com/MOS.
