The head of the penis is outside of the tube along with the cable clamps. the tube inturn pushed against the cable clamps. see the pic

Its a simple operation and very comfortable to wear. you put your penis in the tubes with wrap, attatch the cable clamps at the end of the penis around the wrap and then push the inner tube upwards. you keep the stretch on by placing a bolt or something else through the holes either side.
