Just got the PMP. When is the best time to apply it? When the penis is flaccid, erect or semi-erect?

Is the package discreet? I am looking to order and Extender [and Bathmate as well] I don't want to buy them if Penis Extender or Enlarger is going to be written all over the box [in big font no less]
