For me, intensity has been the key ingredient (along w/consistency)...so always look to up the pressure, (but make sure you don't cross into the pain threshhold...discomfort, yes, pain, no)
Almost 10 years of aggressive pumping have transformed me from (what I thought was) rather pedestrian 6 3/8" BPEL and around 5 1/2" MSEG to 8+/7-, so I can attest to the fact that pumping works.

The actual mechanics of a pump (any pump) is very basic, so I'd check to make certain the quick-release valve at the top of the tube hasn't backed its way out a tad or the hand-pump inner diaphragm hasn't developed a leak. Maybe try shaving the area where the cylinder contacts the skin and...
just make certain that you get the proper size of PVC tubing, IIRC it's 7/8" inside diameter but maybe pring the VV pump to the hardware store to double check. I also warmed-up the tubing a bit to make it slide over the BM tip
picture a 2"-3" piece of clear PVC fitting over the top of the BM tip, then the VV pump just fits at the end of the PVC...pump to your desired pressure and release by using your finger pressing the BM valve
you can also keep the BM 'as-is' and just slide a short piece of PVC to act as a connection point for the VV pump...no need to dismantle anything and no need to use the VV cork
I have discoloration from broken blood vessels caused by pumping aggressively...I chalk-it-up to scars that I've earned. The trade-off is worth it for me, but one of these days I'm going to try Mike's smoker's toothpaste solution and see if that helps

The key is sizing the cylinder and maintaining (safe) pressure...the MityVac is the best device for both maintaining a consistent Hg and varying the pressure during each session.
I'll opt for my BM (modded) once in a while, but my 'go-to' is the MV
